Therapy for Transition- Age Youth

Transition-age youth (typically ages 16–25) often face major changes as they move from high school into adulthood. Therapy for transition-age youth focuses on supporting young people with disabilities as they develop independence, emotional regulation, and confidence navigating relationships, work, school, and everyday life. At Bloom and Bridge Therapy, sessions are designed to help young adults build the skills they need to move forward with greater autonomy and self-understanding.

Therapy for Young Adults with Disabilities

Many young adults with disabilities benefit from therapy that respects neurodiversity while supporting growth and independence. My approach to this is neurodivergent-affirming and strengths-based, focusing on communication, relationships, emotional wellbeing, and navigating adult responsibilities. Together, we work on practical tools that help individuals feel more confident and supported in everyday life.

Life Skills Therapy 

Life skills therapy helps individuals develop the practical abilities needed for everyday independence. This may include communication skills, emotional regulation, daily routines, decision-making, and navigating community environments. Through supportive and individualized sessions, life skills therapy helps young adults with disabilities build confidence while strengthening the skills that support greater independence and wellbeing.

Developing independence is a gradual process that looks different for every individual. Independence-building therapy focuses on strengthening confidence, problem-solving skills, and the ability to navigate daily life more independently. Through real-life practice and supportive guidance, individuals can build the skills they need to move toward greater autonomy, connection, and self-confidence.

Relationships and Communication Support 

Relationships are an important part of building independence and a meaningful life. Relationship and communication support helps neurodivergent individuals and individuals with disabilities navigate friendships, dating, boundaries, and communication in ways that feel authentic and comfortable. Through supportive, neurodivergent-affirming therapy, individuals can explore relationship skills, build confidence connecting with others, and develop tools for navigating social and emotional experiences.

Parent coaching offers supportive guidance for parents and caregivers of children, teens, and young adults with disabilities or neurodivergence. Parenting can bring unique questions and challenges, and coaching sessions provide a space to explore practical strategies for communication, emotional regulation, behavior support, and fostering independence. Together, we focus on approaches that honor each child’s strengths and needs while helping parents feel more confident, supported, and connected in their caregiving role.
